Heart N Soul- Holistic Pilates and Wellness, LLC requires all clientele to agree to and sign a liability waiver and release form prior to participation in all services.

Heart N Soul- Holistic Pilates and Wellness Studio, LLC LIABILITY WAIVER AND RELEASE

In participating in the services offered at Heart N Soul- Holistic Pilates and Wellness Studio, LLC. I agree that I do so at my own risk. In consideration of being permitted to use the facilities and equipment located at Heart N Soul- Holistic Pilates and Wellness Studio, LLC , I understand that Heart N Soul- Holistic Pilates and Wellness Studio, LLC, shall not be liable for the loss or theft of, or damage to, the personal property of a guest or member.

I assume full responsibility for any injuries or damages which may occur using studio facilities and equipment, and I hereby fully and forever release and discharge Heart N Soul- Holistic Pilates and Wellness and Studio, LLC, its owners, employees, and members from any and all claims, demands, damages, rights of action, or causes of actions, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting or arising out of my or a member's use or intended use of the facilities and equipment, on or about the premises of Heart N Soul- Holistic Pilates and Wellness Studio, LLC. I acknowledge that I am fully liable for all medical expenses as a result of any injury incurred during my participation in the services offered at Heart N Soul- Holistic Pilates and Wellness Studio, LLC.

I warrant, represent, and agree that I am in good physical condition and have no disability, impairment, or ailment preventing me from engaging in active or passive exercise that will be detrimental to health, safety, comfort, or physical condition if I do so engage or participate. If my health condition changes, I acknowledge that it is my obligation to take full responsibility for my health, wellbeing, and safety during any type of exercise, and avoid any exercises I have been advised by my physician not to do. I understand that I am not obliged to participate in any activity that I do not wish to do, and that it is my right to refuse such participation at any time.

Consent

I understand and acknowledge that I have the right to consensual practice and that consent is a requirement of tactile feedback and physical touch for all services at Heart N Soul- Holistic Pilates and Wellness, LLC. I understand that I will be given the option in every session to give consent for hands-on adjustments.
